Thyroid nodules with indeterminate cytology: is it safe to say ‘thyroidectomy can be excluded if 18F-FDG-PET suggests benignity’?

چکیده

We read with interest the article by Rosario et al., which summarized practical applicability of accessible imaging method in management thyroid nodules indeterminate cytologically (TNIC) (Rosario, al. 2021). For 18F-FDG-PET, it was stated that a nodule characterized low or absent 18F-FDG uptake is considered at very risk malignance, and thyroidectomy can be excluded if these tests suggest benignity. respectfully want to share different opinion based on our previous work.

Thyroid Nodules with Indeterminate Cytology

To the Editor: Alexander et al. (Aug. 23 issue)1 report findings from a prospective, multicenter gene-expression classifier validation study that raises several questions. For the 265 indeterminate fine-needle aspirates they tested, the geneexpression classifier had a sensitivity of 92%, a specificity of 52%, a positive predictive value of 47%, and a negative predictive value of 93%. Why the ob...

Thyroid nodules with indeterminate cytology: prospective comparison between 18F-FDG-PET/CT, multiparametric neck ultrasonography, 99mTc-MIBI scintigraphy and histology.

PURPOSE To evaluate the role of (18)F-flurodeoxiglucose positron emission tomography/computed tomography ((18)F-FDG-PET/CT) in predicting malignancy of thyroid nodules with indeterminate cytology. PATIENTS AND METHODS We analysed 87 patients who have been scheduled to undergo surgery for thyroid nodule with indeterminate cytology. All patients underwent (18)F-FDG-PET/CT, multiparametric neck ...

Preoperative diagnosis of benign thyroid nodules with indeterminate cytology.

BACKGROUND Approximately 15 to 30% of thyroid nodules evaluated by means of fine-needle aspiration are not clearly benign or malignant. Patients with cytologically indeterminate nodules are often referred for diagnostic surgery, though most of these nodules prove to be benign. A novel diagnostic test that measures the expression of 167 genes has shown promise in improving preoperative risk asse...

Surgeon-performed ultrasound can predict benignity in thyroid nodules.

BACKGROUND Surgeon-performed ultrasound (SUS) has become an extension of the physical examination in the evaluation of patients with thyroid nodules. Amid concerns that too many "diagnostic" thyroidectomies are performed, this study examines the usefulness of SUS in predicting benign thyroid nodules. METHODS A retrospective review of 624 patients with solitary or dominant thyroid nodules and ...

A Genomic Alternative to Identify Medullary Thyroid Cancer Preoperatively in Thyroid Nodules with Indeterminate Cytology

BACKGROUND The use of calcitonin screening for the rare medullary thyroid cancer (MTC) is controversial due to questions of efficacy, accuracy, and cost-effectiveness. This study reports the results of a large prospective validation using a machine-trained algorithm (MTC Classifier) to preoperatively identify MTC in fine-needle aspiration biopsies in lieu of calcitonin measurements. METHODS C...
